Dental HMO’s
These plans are known as “capitation plans,” and operate like medical HMO health insurance programs.  A Dental Health Maintenance Organization (DHMO) is an example of a capitation plan.  The dentist is paid on a per person basis rather than for the actual treatment.  The participating dentists receive fixed monthly fees based on the number of patients assigned to the program.

Preferred Provider Organizations
A preferred provider organization (PPO) allows a particular group of patients to receive care from a defined group of dentists.  The dentist agrees to charge less than their usual fee providing savings for the plan purchaser.  If a patient sees a dentist outside the (PPO) group or “preferred provider” then the cost to the patient will be more and in order for some portion of the fee to be covered by the insurer the procedure must be first approved.

Indemnity Insurance (group coverage)
You may choose your own dentist in this plan type.  The insurance company pays the dentist on a traditional fee-for-service basis.  Patient and employer pay’s a monthly premium to an insurance company and the insurance company reimburses the dentist for the work.  These plans often have pre set deductible amounts and service coverage’s. 

Direct Reimbursement Plans
These plans are self-funded plan and not insurance and are now being offered by employers as an additional benefit for the employee.  Your company might reimburse up to 100% of the first X dollar amount of your dental expenses and then say 80% of the next dollar amount and so on.  Usually any direct reimbursement plan will have an annual cap but no monthly payment or co-payment.  You will be required to pay your dental bill and then you may request reimbursement in accordance with your firms plan.

 Dental Discount
Dental Discount plans are not insurance.  The managing organizations negotiate with local dental offices to establish a set price for specific procedures and pass the savings to the customer.  There are usually no exclusions or waiting periods with these plans.  There may be some restrictions regarding maximum limits of coverage and there are monthly premium requirements.  There are also no co-payments or deductibles with most discount plans.
